Hyatt - 3 field log in issue?

some time recently, www.hyatt.com (hotel chain) started using a 3 field login system, consisting of (a) username, (b) last name, (c) password.

I've tried everything I can think of (including the "Save All Entered Data"), completing deleting the stored history, re-doing, etc.

However, everytime, it seems like LastPass fills in the Username for both fields (a) and (b), even though I can see when I go to my vault that the last name is actually my last name.

This is happening in Chrome and Edge for me.

Anyone else with this issue, and any solution for it?

Please re-save the site by deleting it ( make sure you take note of the username and password) and re-capture it manually with the Save All Entered Data feature.
